Registration Form: Alliance-only Access

Individuals working in a Catholic health care facility or associated with an affiliated Catholic health care organization can apply to become a registered user of the Alliance-only resources on the CHAC website by completing the form below.

In filling out the form, please enter the address of your organization - not your home address. You will receive your UserID and Password by e-mail. Please allow 48 hours for processing of your registration.
